If I understand well, you are trying to translate a picture directly from your post, am I right ?
Just to be sure that I was not missing anything, I replicated all your steps on my test environment.
These are the steps I followed to translate my images:

1. I opened the post I wanted to translate

2. I add a media from the library by clicking on ‘add a media’.

3. Then I checked the options you mentioned in your point number 1:
{x} Duplicate uploaded media to translations and
{x} Duplicate featured image to translations" (I ticked that box too.)

4. I Updated/published the post

5. From the translation box, on the same page of the post, I clicked on the translation of my choice.

6. I clicked again on ‘add media’ and I wrote the alt-text and the description in the language I wanted.

7. I went on my front-end and I could see the media translated according to the language selected.

Does it work better ? Please keep me posted.

In case of you want to translate a media from the library directly, I give you some other tips:

1. Go to the media library admin screen and upload your images. Write down the description, caption alt-text etc.

2. Select a secondary language (from the top of your WP dashboard) and go back to your library.

3. Click to edit the image in one of the secondary languages. Here you can translate things like the caption, description and alt-text.

Finally, you mentioned some other issues in point 3 and 4, may I kindly ask you please to open two new tickets for them? Both of the issues will be solved quicker by our team and this will also help other users with similar problems find solutions when searching the forum.

Normally, once your site is registered in your WPML account and the registration key has been added to the site, you can simply use the automatic updates on the plugins page. If that is not working, then these are the steps to manually update your plugins:

1. Ensure that you have a full backup for the site.

2. Log in to your account and go to the Downloads section. Download the WPML components that you need. You will receive a ZIP file for each one of WPML’s components.

3. Deactivate the current WPML version that you are using. Be sure to deactivate WPML and all its components. Otherwise, the installation files may be locked and not writeable. Don’t worry, your translations are not going to be modified.

4. Delete the plugin, so that you can install the new version. Note: if you cannot delete from inside WordPress, use an FTP program to delete the folders of WPML and its components from wp-content/plugins.

5. In the WordPress admin, go to Plugins -> Add New -> Upload and upload the ZIP files that you downloaded. Alternatively, if you cannot upload plugins directly to WordPress, unzip each file. A directory will be created. Then upload the unzipped directories to your site’s ‘wp-contents/plugins’ directory and overwrite the existing folders for WPML and its components.

6. Log in to your site, go to Plugins and activate WPML core first and then its components.
